The diagnosis and management of polymyalgia rheumatica (PMR) is a challenge. We see lots of people with aches and pains, notably in the age group in whom PMR presents (peak age 70-80 years), the range of potential diagnoses is high, yet getting the diagnosis right is so important given most people with PMR will need 1-2 years of steroid therapy.
A BJGP study from 2018 (BJGP 2018; 68 (676)) discussed some of the challenges we face in primary care, and highlighted two issues which are often poorly considered, notably an awareness of other conditions that can mimic PMR (including malignancies), and the starting dose of prednisolone. The British Society of Rheumatology guideline dates back to 2009/10, so a recent pair of articles in the Lancet (Vol 402, Iss 10411 Oct 21, 2023) and NEJM (N Engl J Med 2023; 389:1328-1329) give a welcome update, with positive news about potential future therapeutics for this difficult to treat condition.
